| Police deliver agenda by hand | Existing cases Prepares a file containing the case notes of those for discussion at that month’s panel. Note if it is a long-term case only the notes from the previous two panels are attached, old papers remain on master file. New cases Check new cases against PI system. Inform Head of Service (Performance Audit and Review) if there is a current social worker for the case or if it was previously known and closed, giving locality social work team details and case paper ref number. Also check ICS and print off Case Overview Sheet (COS). |

All case notes and papers are kept in a secure locked cabinet with access by CPU only
NOTIFICATIONS RECEIVED FROM HM PRISON ON THE RELEASE OF A PRISONER TO AN ADDRESS IN THIS BOROUGH
- CPU Business Support Officer sends standard letter of acknowledgement back to the prison
- If the case is open to Family Services, send a copy of the notification and our letter to the appropriate locality social work team with a note saying for information
- If the case is not previously known CP Business Support Officer to add a MAPPP warning to PI and prepares the case for MAPPP. No copy to locality social work team.
